Application For Daycare/Babysitting Transportation – Instructions
This form is for families with court-ordered shared custody of a student. Both parents must live within the borders of North Allegheny School District to have an additional stop The form can be found in the BusHelp portal located at:

On the portal you will find an icon labeled “Get Help”.
Click on “Get Help”. This will open the ticket entry page as follows:
Enter your e-mail address in the Requester field and then click on the “Reason for ticket” dropdown list. At the bottom of the list, you will find the “Forms” selection, click it.
This will reveal another dropdown list of forms to be requested:
Click on “Application For Daycare/Babysitting Transportation”
This will reveal the form for entering information needed to process your request. All information that is required to process your form in a timely manner will either have a little red asterisk * after it or a notation that states (Required). Failure to provide the information may prevent the form from being submitted.
The first section asks for the student’s information including “First Name”, “Last Name”, “Grade”, and “Address”.
Select which district school your student attends:
When you select your student’s school another dropdown list – “Daycare Facility” – is shown:
For example, Hosack will display Daycare Facilities such as:
At the bottom of the list will be an option to provide a facility that is not listed, or the address of a Babysitter:
If “Other” is selected, you will be asked to enter the name and address:
Next, please specify transportation directions being requested:
The next section of the form captures your parent/guardian information. Please note that the phone number should include hyphens for ease of readability. For the email address please enter this if it differs from the requester email above.
The next section of the form captures when this transportation will be in effect:
When all the information has been entered, then click the “Submit” button at the lower right of the ticket form:
An email will be sent to you with a summary of the request.
