Act 372 Request for Transportation to A Non-Public School – Instructions
This form is used to request transportation to a non-public school. The form can be found in the BusHelp portal located at:

On the portal you will find an icon labeled “Get Help”:.
Click on
“Get Help”. This will open the ticket entry page as follows:
Enter your e-mail address in the Requester field and the click on the “Reason for ticket” dropdown list. At the bottom of the list, you will find the “Forms” selection, click it.
This will reveal another dropdown list of forms to be requested:
Click on “Act 372 Request for Non-Public School Transportation”
This will reveal the form for entering information needed to process your request. The first section asks for the student’s information including “Student Status” (New, Existing, etc.), “First Name”, “Last Name”, and “Date of Birth”. All information that is required to process your form in a timely manner will either have a little red asterisk * after it or a notation that states (Required). Failure to provide the information may prevent the form from being submitted.
The next section of the form captures the gender of your student. This helps Transportation make sure they address your student correctly (he/she/they) if an agent needs to speak with you about your request.
Next, select what grade your student attends.
If you select any of the Kindergarten options a note will appear reminding you that transportation for kindergartners is only provided one way.
The next section of the form captures the student's address.
Next, select the non-district school that your student will attend:
The next section of the form captures your parent/guardian(s) information. Please note that the phone number should include hyphens for ease of readability. For the first parent’s email address please enter this if it differs from the requester e-mail above.
Next, select the portion of the day for you which you are requesting transportation and when the transportation needs to begin and end.
Then, please list any allergies that your student may have:
Finally, you must acknowledge the residency requirements to be eligible for transportation to be provided by the school district.
When all the information has been entered, then you can click the “Submit” button at the lower right of the ticket form:
The form must be submitted by July 1st prior to the start of school in order for Transportation to perform all of the necessary routing and bus assignments for you and the hundreds of other students who will enjoy NA transportation to and from school.
